If you areregistering as a patient on the Services: we may collectpersonal information, such as your name, email address, phone number, date ofbirth, gender, zip code and location, services and procedures about which youinquire, identification (e.g., a photo of your government-issued ID) or otherauthentication information, and payment information, when you register for oruse our Services, build a profile on our Services, sign up for our mailinglist, request an appointment with a healthcare provider to receive a ClinicalService, or otherwise communicate with us. The supply of such informationto Company is at your discretion. We may also collect any communicationsbetween you and Company and any other information you provide to Company.
If you areregistering as a physician on the Services: we may collectpersonal information, such as your name, email address, phone number. We willalso collect certain professional information in order to verify yourcredentials.
If you areregistering as a nurse on the Services: we may collectcertain personal information, such as your name, email address, phone number.We will also collect certain professional information in order to verify yourcredentials.
Data collectedthrough the use of the Services. We collect information abouthow you use the Services, your actions on the Services, and content you post tothe Services, and any content you provide through the Services (“User Content”). Please rememberthat Company may, but has no obligation to, monitor, record, and store UserContent in order to protect your safety or the safety of other users, to assistwith regulatory or law enforcement efforts, or to protect and defend our rightsand property. By using the Services, you consent to the recording, storage anddisclosure of such communications you send or receive for these purposes.
Information wereceive from third party sites you connect to our Services. Wemay receive personal information about you from third parties and combine thatwith information we collect through our Services. For example, we may obtaininformation from the health care provider who is administering the ClinicalService for you. We may also obtain information when you login through a thirdparty social network or authentication service, such as Facebook or Google.These services will authenticate your identity and provide you the option toshare certain personal information with us, which could include your name,email address, address book and contacts, or other information. Similarly, when you interact with us through a social media site or third partyservice, such as when you like, follow or share Company content on Facebook,Twitter, Pinterest, Instagram or other sites, we may receive information fromthe social network including your profile information, picture, user IDassociated with your social media account, and any other information you permitthe social network to share with third parties. The data we receive from thesethird party sites is dependent upon that third party’s policies and yourprivacy settings on that third party site. You should always review, and ifnecessary, adjust your privacy settings on third party websites and servicesbefore linking or connecting them to our Services.

Wemay de-identify information collected from and about you and use and disclosesuch de-identified data for any purpose.
We,and our third party partners, automatically collect certain types of usageinformation when you visit our Services, read our emails, or otherwise engagewith us. We typically collect this information through a variety of trackingtechnologies, including cookies, web beacons, embedded scripts,location-identifying technologies, file information and similar technology(collectively, “tracking technologies”). To the extent we create, receive, maintain ortransmit protected health information on behalf of a health care provider as a“business associate” as such term isdefined under HIPAA, we will only use and disclose your “protected health information” as permitted under HIPAA. To theextent permitted under applicable laws, we may share your personal informationwith:
Health care providersadministering Clinical Services. We may share certain information aboutyou, such as your name and address, with the health care providers who willadminister Clinical Services to you. Before proceeding with any such ClinicalServices, the nurse who will administer the Clinical Services on behalf of thephysician practice will obtain your consent using the practice’s form.
Third parties at yourrequest. For example, you may have the option to share your activities on the Serviceswith your friends through email, text or on various social media sites;
Third-party vendorsand other service providers that perform services on our behalf, asneeded to carry out their work for us, which may include processing payments,providing mailing services, providing tax and accounting services, web hosting,or providing analytic services;
The public when youprovide feedback or User Content on our Services. For example, if youpost User Content on our blog or comment on our social media sites, yourinformation, such as your username and your comments, may be displayed on ourwebsite or on our social media pages;
Other parties inconnection with a company transaction, such as a merger,sale of company assets or shares, reorganization, financing, change of controlor acquisition of all or a portion of our business by another company or thirdparty, or in the event of a bankruptcy or related or similar proceedings; and

4. CONTROL OVER YOUR INFORMATION
Profile and Data Sharing Settings. You may update your profile information, such as your username, and may change some of your data sharing preferences by visiting“Edit Profile” in your account.
Access to your Device Information. You may control the Services’ access to yourdevice information through your “Settings”app on your device. For instance, you can withdraw permission for the app toaccess your location, photo stream and camera.
How to control your communications preferences: You can stop receiving promotionalemail communications from us by clicking on the “unsubscribe link” provided in such communications. We makeevery effort to promptly process all unsubscribe requests. You may notopt out of service-related communications (e.g., account verification,transactional communications, changes/updates to features of the Services,technical and security notices).
Modifying or deletingyour information: If you have any questions about reviewing,modifying, or deleting your information, or if you want to remove your name orcomments from our website or publicly displayed content, you can contact usdirectly at firstname.lastname@example.org. We may not be able to modify or delete yourinformation in all circumstances.
5. HOW WE STORE AND PROTECT YOUR INFORMATION
Data storage andtransfer: Your information collected through our website may be stored and processed inthe United States or any other country in which Company or its affiliates orservice providers maintain facilities.
